To register please come into the surgery and fill out a form. You will need to provide some form of identification, (eg a passport or driving licence).
When you have registered you will be invited for a new patient medical with the Practice Nurse or a doctor, which will include a general health check. Personal and medical information is kept on computer at the surgery and is only shared with those who need it for medical reasons. Your demographic details (which consist of your name, address, date of birth and NHS number) are also on the National Care Records Service or “Spine”.
